social_workers_2     Mary Kay Evans, Director of Social Services for Care Pathways has been a social worker with 24 years of experience in medical social work, counseling and consulting within the Orange County area in acute care, skilled nursing and private practice. Ms. Evans is well known in the community for her commitment to families and seniors. She opened the first Transitional Care Unit at Fountain Valley Regional Hospital in Orange County in 1987. She has worked to further the education and training of long-term care facilities when she founded Care Pathways

in 1999. Her commitment to our community of seniors and families in working as a unit with other health care professionals has brought a higher standard to the level of care within our community of care providers and facilities.  read more...

 Based upon the idea of keeping seniors and family informed when someone is facing a medical crisis, our vision was to create a service that would not only ease the stress of caring for a elderly loved one but also to provide communication, and a means for people to connect with one another in a safe warm and friendly environment with other caregivers who support and can share their stories as well can ask questions of out team of social workers.
 
• The mission of Care Pathways is to amplify the information, hope and compassion for those senior's and families in need of real time answers when faced with caring for a loved one. Offering options in areas of long or short term care as well as a network of services that is available for Southern California seniors.

 

• Our vision is to carry this information throughout Southern California with a network of care providers and services for seniors and those who care for them. Care Pathways was established in 1999 by Mary Kay Evans Director of Social Work. Her vision in caring has assisted and maintained the high quality control of care for the senior population with her many areas of expertise for over 30 years.  Care Pathways is a senior care agency providing free information and services that connect seniors experiencing  health challenges while supporting family and friends through this process and beyond
